- Recent 1x1 ID Photo
- PSA Birth Certificate (for NEW STUDENTS)
- Previous Report Card with LRN/PEPT Result (for NEW STUDENTS)
- Latest/Updated Diagnosis Report and Recommendation to be mainstreamed/homeschooled for the School Year from the student's Developmental Doctor (for students with special needs)
